The paper introduces the clinical experience of professor SUN Shentian in treatment of qiaobi (orifice obstruction) with acupuncture. In pathogenesis, qiaobi refers to the obstruction of qi movement and abnormal ascending and descending of qi. "Obstruction of zangfu orifices" is the biao (symptoms), while "obstruction of brain orifice" is ben (root cause) of diseases. In the principle of treatment, bian and ben should be considered simultaneously, and it is proposed to regulating the spirit and unblocking the orifices in treatment. Baihui (GV 20) and the cognitive emotional area on the head are selected for regulating the spirit and opening the brain orifice, focusing on the root cause of diseases. Based on the connection between the symptoms and the cerebral cortical functions, the different points on the head are used, the Siguan points (Hegu [LI 4] and Taichong [LR 3]) and yuan-source points combined, and the acupoint prescription is composed according to the interior-exterior relationship of meridians. With repetitive transcranial acupuncture stimulation at the points on the head, the therapeutic effect is strengthened. It provides a novel approach to treatment of diseases related to qiaobi.

This paper introduces professor SUN Shen-tian's clinical thoughts and his characteristics of acupuncture techniques for the treatment of depression based on "psychosomatic medicine". Professor SUN, the master of traditional Chinese medicine, believes that depression refers to comorbidity of "heart mind" and "body", resulting from the "body-mind" disharmony, specially dominated by the emotional disorder. This disease is located in the brain, with the injury of mind and closely related to the heart and liver dysfunction. In pathogenesis, the dysfunction of brain mind and the unhealthy conditions of body and mind are involved. The treatment should focus on "regulating the mind, improving the intelligence, co-modulating the abdominal and brain functions and treating the physical and mental disorders". Baihui (GV 20), Ningshen (Extra) and emotional area on the head are selected as the main points to benefit the intelligence and calming down the mind; the abdominal region 1 and region 8 of "Sun's abdominal acupuncture" are used as the main points of the abdomen to regulate the brain functions. The point prescription is modified according to the symptoms and etiologies. The repeated transcranial acupuncture stimulation and electroacupuncture at low frequency (2 Hz) are crucial to the therapeutic effect. Reliving anxious emotions is specially considered before acupuncture, and the mind is protected and deqi is consolidated during acupuncture.

To introduce the clinical experience of professor SUN Shen-tian in treatment of Tourette's syndrome (TS) with acupuncture. TS is a psychosomatic disease and the core pathogenesis refers to blood deficiency producing internal wind. The disease is located in the heart and liver. Acupoints are selected according to the functional orientation of the cerebral cortex. The extrapyramidal system area is preferred for tic disorder, and the emotional area is for behavioral disorder. The treatment focuses on regulating the mind by multiple methods, including manual needling technique used the transcranial repeated acupuncture, and regulating the mind by taking multiple acupoints, Baihui (GV 20), Neiguan (PC 6), Shenmen (HT 7) and Dazhong (KI 4) are stimulated. For abdominal twitching and mental symptoms of TS children, the first and third abdominal areas are selected. The target symptoms (biao) are treated specially by local acupoints, the combination of the starting and ending acupoints of the affected meridian, or the acupoints of the meridians with same name. The modified chaihu longgu muli decoction and siwu decoction are prescribed to sooth liver, nourish blood and soothe wind. In association with the characteristics and target symptoms of TS, the sequential therapy is used with filiform needling, intradermal needling, Chinese herbal medication and psychotherapy.

SUN applies meridian syndrome differentiation to the diagnosis and treatment of diseases; advocates that prevention and treatment of diseases should be regulated mind firstly; applies transcranial repetitive acupuncture combined modern cerebral cortex function positioning; emphasizes the application of multiple acupuncture methods and manipulation, and includes the meridian penetrating needling method, the flat needling and penetrating needling method, and the stagnant needle lifting method, pays attention to the importance of achieving qi and manipulation for the effect.

Professor SUN Shen-tian's academic thought and clinical experience of acupuncture for peripheral facial paralysis are summarized. According to the different stages of facial paralysis, professor SUN 's treatment plan has its own emphasis. In the acute stage, professor SUN emphasizes the importance of standardized western medicine treatment, and proposes early acupuncture treatment. The treatment mainly focuses on dispersing wind and cold, promoting blood circulation and dredging collaterals. In the recovery stage, professor SUN emphasizes the method of "pulling the sticking needle", and applies repeated transcranial acupuncture according to the functional orientation of cerebral cortex. In the sequelae stage, professor SUN emphasizes the value of regulating the mind. In the course of treatment, professor SUN highly values the use of flexible acupoint selection, repeated transcranial acupuncture, "pulling the sticking needle" method, acupoint pressing and giant-needle method, combined with electroacupuncture and targeted injection. In addition, based on the clinical experience, professor SUN summarizes the common methods to estimate the prognosis of peripheral facial paralysis.

RESUMO

The patient was a 27-month-old girl a congenital defect involving the 47th chromosome who was referred to an acupuncture clinic for supportive treatment of Trisomy-18 (T-18). The condition results in severe medical complications, developmental delays and a shortened lifespan. This patient was unable to eat or swallow, unable to move, and had difficulty sleeping. Her parents and the flew to New Mexico from New York and the patient underwent four consecutive neuro-acupuncture treatments (also called scalp acupuncture). The treatment response was immediate resulting in dramatic sleep improvement, cessation of vomiting, beginning to eat, and improved ability to stand and subsequently move. Treatment effects have been sustained for more than one year.

OBJECTIVE: To compare the effect on post-stroke insomnia between the repetitive transcranial acupuncture stimulation (rTAS) and the conventional western medication in the patients and to explore the mechanism. METHODS: Ninety patients of post-stroke insomnia were randomized into a rTAS group, a medication group and a placebo group, 30 cases in each one. In the rTAS group, patients were intervened by rTAS. The acupoints were Baihui (GV 20), Ningshen (Extra), emotion area, Wangu (GB 12), Taiyang (EX-HN 5), Neiguan (PC 6), Shenmen (HT 7), Sanyinjiao (SP 6), Zhaohai (KI 6), Zusanli (ST 36) and Taichong (LR 3). Fast twist with small amplitude was used at Baihui (GV 20) and emotion area for 2-3 min, 200-300 r/min, once 15 min. Electroacupuncture (EA) was applied at Baihui (GV 20) and Ningshen (Extra), bilateral Wangu (GB 12), Sanyinjiao (SP 6) and Zhaohai (KI 6) on the same side, 10 Hz, 0.5-1 mA. The treatment was given for 40 min in the rTAS group, once a day. Diazepam was prescribed orally in the medication group before sleep, 2.5 mg a day. Starch capsule was used in the placebo group before sleep, once a day. All the treatment was given for continuous 1 month. The level of serum orexin A was observed before and after treatment. The effects were compared. The recurrence rate was recorded 3 months after treatment. RESULTS: The total effective rates in the rTAS group and the medication group were 86.7% (26/30) and 90.0% (27/30) repectively after treatment, which were better than 20.0% (6/30) in the placebo group (both P<0.01). After treatment, the levels of serum orexin A in the rTAS group and the medication group were lower than those before treatment (both P<0.01), which were lower than that in the placebo group after treatment (both P<0.01), without statistical significance between the rTAS group and the medication group after treatment (P>0.05). The total effective rates in the rTAS group and the medication group were 86.7% (26/30) and 86.7% (26/30) at follow-up repectively, which were better than 16.7% (5/30) in the placebo group (both P<0.01). CONCLUSION: The rTAS is safe and effective for post-stroke insomnia, which is similar to oral medication of diazepam. The decreasing serum orexin A may be one of the mechanisms.

Effect of acupuncture on free radicals after spinal cord injury was observed in rats with experimental spinal cord injury (SCI). Results indicated that within 24 hours after SCI malondialdehyde (MDA) increased progressively, 2 hours after SCI it reached the peak; and the superoxide dismutase (SOD) activity decreased significantly at the same hours, the decrease being the most marked 2-6 hours after SCI. The MDA content in the acupuncture group was significantly lower (P < 0.05) and the SOD activity higher (P < 0.01) than that of the control group respectively. It is suggested that acupuncture inhibits production of MDA and increases the SOD activity.
